Via Tender: Implement a 'report' button

Assigned to Tender discussion #105. As reported in Tender:
On fanart.tv we use a report button when we find that an image isn't right.
We as moderators can browse through the all reports and adjust when necassary.This would be a great addition to the website and much easier to handle than the current way of reporting through the forum.
This way you can report duplicates/tvshows bad images. Movie info screen that don't function (missing text fields).
Ofcourse the user has to fill in some required info or choose from standard options.
