I believe the title is "reversed" :-)
It should be: Re-factor translations to support multiple countries per language (en-US, en-GB, pt-PT, pt-BR, etc...)
ISO 3166-1 country codes should be added to differentiate languages represented today with ISO 639-1 language codes.

Hi all,
I see that this is becoming a nightmare for those who have a different language. My case for example.I would love to ask you to add as quickly as possible the PT-BR language (Portuguese-Braziliam) because the problems are already increasing.
Several titles are being changed with the PT-PT standard (Portuguese-Portugal) with wrong names for my language, and it is not possible to change them ... https://www.themoviedb.org/talk/563f70f0925141344200709a
Are there any plans to add the PT-BR language in TMDB?

Yes, there are plans (that's what this ticket is about). I'm hoping to get to it in the new year. The new website is fully localized and this will wrap this up from start to finish for the data on TMDb.
